Reports To: Building Principal & Director of Student Services

ISBE Vision / Hearing Screening Certification is preferred but not required. Training will be provided.

Duties:

1. Provide medical services and support to elementary-age students 

2. Complete medical history reports for special education IEP and 504 plans

3. Attend IEP Eligibility, 504 Plan, and some IEP meetings

4. Manage Student Health Data and required health information

5. Complete annual hearing and vision screening (If necessary, ISBE training will be provided)

6. Complete and report head injury/concussion paperwork

7. Service on school and district committees

8. Communicate with parents

9. Report student health and social-emotional concerns to the principal or appropriate supervisor

10. Distribute and record the distribution of the medication to appropriate students

11. Provide health care training to staff

12. Provide parents with names of outside healthcare resources

13. Maintenance of AED Machines 

14. Monitor staff CPR/AED machine training

15. Develop and maintain student health plans
