Hope Learning Academy is seeking an experienced full-time Case Manager to lead our team of educational professionals in designing and providing services to our diverse learners.   JOB SUMMARY: The case manager ensures that services in the Individual Education Plans (IEPs) are provided as described with fidelity. All state and federal timelines and protocols are met. IEPs are written to provide the supports needed for students to thrive and grow. Data is used in all decision making. Professional Development is provided to ensure all team members are writing high quality IEPs and providing effective services. Plans are designed to promote the physical, social, behavioral, academic, and emotional development of the students of Hope Learning Academy. Most of all, you are driven by the belief that all students will succeed. 

  

ESSENTIAL FUNCTIONS:  

· Develop, oversee, and coordinate Special Education services.  

· Develop a yearly meeting schedule that ensures 100% compliance in accordance with special education due dates and timelines.  

· Proactively communicate with staff members to ensure all members of the IEP team are prepared to meet all IEP and 504 deadlines.  

· Schedule, complete, and document all required parent communication in preparation of IEP and 504 meetings in compliance with state and federal laws.  

· Lead IEP and 504 meetings as a member of the IEP team.  

· Support school teams in collecting and compiling data for all services in the IEPs.  

· Maintain student paper and electronic files in accordance with CPS file protocols and state and federal laws.  

· Review all IEPs for quality and provide feedback and professional development to teaching staff regarding IEP writing.  

· Lead team in developing appropriate transition plans, behavior plans and homebound services for students as needed. 

· Collaborate with test coordinator and special educators to provide appropriate accommodations and modifications during test. 

· Collaborate with the school team to ensure schedules promote success and inclusion. 

· Maintain collaborative relationships with students, staff, families and district personnel. 

· Collaborate with MTSS team to recommend interventions and review data. 

    

WORKING CONDITIONS:  

Work is performed primarily in an indoor office or classroom environment, free from extreme weather conditions. Work may include a moderate amount of noise (business office with computers, phone and printers).  

  

KNOWLEDGE, SKILLS AND ABILITIES: 

· Demonstrated understanding of the individual needs of students with disabilities from a variety of language and cultural backgrounds 

· Strong knowledge of federal and state laws and regulations regarding the delivery of special education and related services 

· Knowledge of intervention and assessment strategies for disabled and non-disabled students 

· Knowledge of multi-tiered systems of support, MTSS 

· Ability to establish and maintain effective working relationships with administrators, staff, students, parents, governmental and community agencies, and employers 

· Ability to problem-solve, work on a team, use data to guide decisions and clearly communicate to multiple stakeholders 

· Demonstrated expertise in consensus building, and use of problem solving skills and strong written, verbal, presentation, and interpersonal communication skills 

  

  TRAINING, ACADEMIC AND EXPERIENTIAL REQURIEMENTS: 

· Completion of a Bachelor’s or Master’s degree in special education or a Master’s degree in educational administration  

· A valid Professional Educator License (PEL) in one of the following areas: Special Education (PK-Age 21) endorsement or School Support Personnel endorsement with a supervisory endorsement  

· Two or more years of experience in special education 

· Expertise in special education curriculum and service provision and knowledge of general education curriculum
