Transition Coach - Full-Time

Job Summary:
The Pre-Employment Transition Services (Pre-ETS) Transition Coach provides direct, hands-on coaching and support to students with disabilities, ages 14-22, to assist them in building the skills, confidence, and experiences necessary to successfully transition from high school to post-secondary education, training, or competitive employment. The Transition Coach delivers individualized and group-based Pre-ETS services in educational and community settings.
Essential Duties and Responsibilities:
Provide one-on-one and small group coaching to students with disabilities participating in Pre-ETS services.
Deliver Pre-ETS instruction in the following areas:
Job Exploration Counseling
Work-Based Learning Experiences
Counseling on Post-Secondary Education Opportunities
Workplace Readiness Training
Instruction in Self-Advocacy
Support students in developing job readiness skills, including communication, time management, teamwork, and problem-solving.
Assist students in identifying career interests and post-secondary goals.
Provide job coaching during work-based learning experiences, including internships, job shadowing, and volunteer opportunities.
Model and reinforce workplace expectations such as punctuality, appropriate attire, and professional behavior.
Collaborate with Pre-ETS Case Managers, school personnel, employers, and families to ensure coordinated service delivery.
Transport students to job sites, college tours, or community events as needed (if permitted by program structure).
Maintain accurate records of coaching sessions, student progress, and participation in activities.
Participate in training, staff meetings, and ongoing professional development.
Promote self-advocacy and independence for students as they prepare for adult life.
Qualifications:
Education & Experience:
High school diploma or GED required; Associate's or Bachelor's degree in Human Services, Education, or a related field preferred.
Experience working with youth, preferably individuals with disabilities or transition-age students.
Previous experience providing job coaching, youth mentoring, or employment readiness services is desirable.

Patience, flexibility, and strong interpersonal skills.
Ability to model appropriate workplace behaviors and expectations.
Strong communication and problem-solving skills.
Ability to work both independently and as part of a team.
Reliable transportation and willingness to travel within Bradley County.
Basic computer skills for documentation and communication.
