MINIMUM QUALIFICATIONS (for new hires at job entry)
Education and Experience: Associate’s degree or completion of 60 college credits or completion of District-approved exam,
and two years of experience; or an equivalent combination of directly-related education and experience.
Required Certifications/Licenses: • CPR/First Aid through "in Person" training - Certification. • TB test. IVP Clearance Card

FingerPrint Clearance
TB Test results within the last 12 months
Personnel Records Form
AZDHS Criminal history affidavit
DES/DCS Department of Child Safety Background Check-Direct Registry Record Form

Signature of being vaccinated
