Description

Summary/Objective: The transition coordinator will develop and support transition activities for college, career and community readiness. The Transition Coordinator assists students with disabilities and their families in obtaining services necessary to transition from high school to a post-secondary setting, whether for post-secondary education, employment, or long-term support.

These services, which are based on each student's strengths, interests and preferences, support students with exceptional needs transition from school to post-school activities, including college education, vocational training, employment, continuing and adult education, adult services, independent living and community participation.

Essential Functions Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions:

Respond to all internal and external customers, as it relates to position, in a prompt, efficient, friendly, and patient manner Manage the Post School Outcomes survey In conjunction with the special education teachers develop goals to meet the students transition needs for education, employment and/or independent living Meet with individual students to discuss their transition plans Review, edit and complete student transition assessments yearly Analyze data from transition assessments to determine student’s strengths and needs for employment Collaborate with counseling and CTE to make sure students are taking electives/classes that align with their transition goals Provide consultation and training to staff in understanding the needs of the students as well as the implementation of the IEP or 504 including goals, accommodations, and/or modifications necessary for employment Facilitate referral, application, eligibility for students and families to adult services such as the Office of Rehabilitation, Department of Developmental Disabilities Develop parent training to prepare students for transition to career and college Develop relationships with Community Colleges and Universities and facilitate relationships with community partnerships for job training Participate as an advisor, resource specialist, and facilitator in faculty, grade level, department, and/or professional learning community meetings Assist teachers and support staff in using research-based instructional and learning strategies and content specific, differentiated instruction including assisting teachers in planning, delivering, and assessing lessons. This might include providing consultation to school staff working with students with disabilities in the following areas: instruction, assessment, curriculum, communication, behavior, organization, use of visual systems, self-management, sensory needs, social skills and the design of the physical environment in the classroom Maintain knowledge of and adhere to all policies, regulations and rules Perform related duties as assigned in a safe and prudent manner Other Duties This job description is not a comprehensive listing of duties, responsibilities, or activities that are required of the employee for this job.
