Description
DEFINITION: .Under general supervision, works on transition activities with secondary students with disabilities in the least restrictive District placement under the direct supervision of the Exceptional Education Director and in collaboration with the TSW Coordinator.
ESSENTIAL FUNCTIONS:

TASKS:
Coordinate and execute assessments and activities for career exploration.
Assist students with identifying and accessing a path to their chosen career/employment goal.
Gather and organize information for transition planning.
Facilitate and support student involvement in school and community experiences.
Supervise and maintain general control of students during transition activities at school sites and in the community.
Ensure parents and service providers are informed of transition activities and are invited to participate with students as appropriate.
Provide information, support, and communication with case managers, teachers, parents, and families.
Notify TSW program coordinator of the special needs of students who display characteristics that vary from the norm. Collaborate with specialists to assist these students as needed.
Locate and share resources for education, training, employment, and/or independent living for transitioning youth.
Coordinate annual Transition/IEP review meetings for seniors.
Facilitate the VR referral process by gathering and submitting necessary documentation as directed by coordinator.
Support VR clients in the development of their Individual Plan of Employment prior to enrollment in TSW courses.
Collaborate and coordinate with outside agencies to facilitate participation in transition planning meetings.
Develop and implement data collection methods to monitor student progress towards postsecondary goals.
Maintain strict adherence to confidentiality regarding all student history and records.
Attend district meetings and trainings as required.
Attend Vocational Rehabilitation/ADE meetings, trainings, and seminars regarding TSW procedures and practices
Follow district and departmental policies and procedures.
Perform other duties as assigned by supervisor.
Assist on collecting data for annual Post-Secondary Outcomes surveys.
KNOWLEDGE, SKILLS AND OTHER CHARACTERISTICS:
Knowledge of District policies and procedures.
Knowledge of developmental, physical and mental delays and disorders.
Knowledge of transition requirements and age of majority requirements.
Knowledge of special education or special needs accommodations.
Knowledge of post-secondary options offered by community agencies, service providers, training academies, community colleges, and universities.
Ability to drive to transition activity sites.
Ability to establish and maintain positive, professional, and effective working relationships as necessitated by work assignments.
Ability to read and interpret eligibility requirements for service and educational programs.
Ability to write routine reports and correspondence.
Ability to speak effectively to employees of the organization and to various agency representatives in the community.
Skill in reading and interpreting documents such as safety rules, operating and maintenance instructions, and procedure manuals.
Skill in writing routine reports and correspondence.
Skill in basic math and computing rate, ratio, and percent and to draw and interpret bar graphs.
Skill in applying common sense understanding to carry out instructions furnished in written, oral, or diagram form.
Skill in dealing with problems involving several concrete variables in standardized situations.
Skill in establishing and maintaining effective working relationships.
PHYSICAL REQUIREMENTS: Work is performed in an indoor environment. Work requires frequent sitting, standing, bending, repetitive motion, vision to monitor, may require some lifting.
QUALIFICATIONS:
Bachelor’s degree from an accredited educational institution in a related field.
Valid Arizona IVP Fingerprint Clearance Card
PREFERED QUALIFICATIONS:
Master’s degree
Special Education Certification
Experience teaching or working with high school age students.
Experience working with people with disabilities.
Depending on the needs of the District, some incumbents in this class must be able to demonstrate fluency in both Spanish and English as a condition of employment.
