JOB TITLE: School Social Worker IMMEDIATE SUPERVISOR: Director of Special Education and Related Services GENERAL DESCRIPTION: Performs difficult professional work providing a broad range of school social work and special education services, and does related work as required. Work is performed under the regular supervision of the Director of Special Education and Related Services. ESSENTIAL FUNCTIONS: -Identifies the legal signing party for special education referrals (Biological Parent, Court Appointed Guardian, etc.) and reviews custody forms; conducts surrogate training and assigns surrogates as needed for students. -Consults with case managers, psychologists, and diagnosticians to schedule meetings within specified timelines. -Schedules appointments with all appropriate parties for initial special education referrals; meets with parents for initial special education referrals to explain parental rights and completes all required forms; conducts the interview of parents for Social History Report; prepares report for school file. -Obtains release of information for any outside agency reports required for eligibility consideration; maintains regular log of all new referrals and routes referrals to school psychologists and diagnosticians. -Attends meetings as a representative of the school division, including child study meetings, student support meetings, initial eligibility meetings, triennials, and department meetings. -Writes eligibility minutes for all initial referrals in a timely manner, including all required VDOE forms. -Prepares, collects and updates all supporting documentation for triennial review including, but not limited to, social history reports and teacher forms documenting environmental issues. -Creates new forms as necessary to ensure regulations are followed and appropriate information is collected. -Accepts referrals from schools for Prek-12 environmental issues for children, including poor hygiene, attendance problems, possible abuse, possible neglect, etc. -Conducts home visits and parent meetings as necessary to assist with environmental issues that are hindering school success, and interviews students as needed. -Provides truancy support under the direction of the Director of Student Support Services. -Makes referrals to the department of social services and/or any local agency to seek services for children/families. -Conducts classroom discussions regarding social issues as requested by teachers. -Attends and serves on local agency boards for families and students such as, the Family Assessment and -Planning Team (FAPT); completes all necessary FAPT training and paperwork. -Petitions the court system when necessary for child safety issues, and attends court hearings as required. -Advises principals and school counselors of appropriate responses to social issues, and conducts county-wide training on social issues when instructed. -Facilitates new services when necessary to assist HCPS families. -Performs all clerical duties as needed to meet job requirements. -Performs other duties as required.

ECSE Preschool Coordinator: -Accepts all verbal or written inquiries regarding ECSE services, including transfer students with an IEP; accepts initial ECSE referrals from parents, physicians, outside agencies, screenings. -Schedules yearly ECSE screenings to meet Child Find Regulations; consults with speech coordinator to arrange coverage. -Attends all ECSE Child Find Screenings and administers age appropriate Brigance Screener to identify children with potential delays. -Completes child study paperwork following all screening appointments; maintains log and files results of all student screenings. -Processes and routes referrals to speech pathologists for speech-only preschool referrals. -Identifies possible school placement for students requiring full ECSE evaluation. -Maintains current caseload numbers for current teachers and ongoing projections for upcoming academic year to determine staffing needs to ensure compliance. -Attends required transition visits to explain early childhood special education services to parents. -Serves as the division consultant at Head Start and on the Piedmont Regional Infant/Toddler Team. -Interviews parents to obtain permission for evaluations and complete all required paperwork, including social history. -Observes referred children in a natural setting, and prepares required eligibility reports. -Prepares and routes referral packets as required. -Consults with service providers as needed. -Attends initial eligibility meetings; prepares written minutes of prior meetings; attends IEP meetings as requested by parents or teachers. -Serves as primary consultant to teachers regarding classroom or parental issues. -Develops and manages the Preschool 619 grant; processes and approves orders utilizing grant funds. -Attends VDOE required LEADS Conference twice a year; attends professional development as needed. -Collects and maintains documentation for reports mandated by the VDOE, including yearly indicators and all data reports to verify compliance. -Schedules yearly review meetings for appropriate kindergarten placements for students aging out of preschool and submits potential IEP referrals as needed. -Trains teachers and local agencies on developmentally appropriate skills, procedures, expectations, and updates for the ECSE program. -Conducts observations in local childcare settings as requested for students with potential disabilities, conducts observations in VPI classrooms when requested for student issues. -Performs other duties as required.
