Summit Academy Support Services Social Worker – Scope of Service
Location: Varies Hours: 20 hrs per week Employment: Part-time
The support services social worker collaborates with support services coordinators to provide direct counseling services to students with IEP goals related to social emotional learning. The social worker will collaborate with the BCBA, Social Emotial Learning Coordinator, and behavior technicians in developing and supporting the delivery of specially designed instruction.
Duties and Responsibilities • Be informed of students with direct and indirect services • Aid in development of tracking sheets using Google Docs, at the beginning of the year and semester • Develop a counseling services schedule and provide a copy to the principal and SSC • Provide direct counseling services to students with IEP Goals in coordination with parent input • Track service minutes and student progress following each counseling session • Write quarterly progress reports related to students IEP Goals; due one week before the end of the quarter • Assist with gathering data for referrals to special education and three year evaluations (i.e. classroom observations, checklists, standardized assessments, interviews to contribute to a student’s developmental history, etc.) • Participate in writing PLAAFP (Present Level of Academic Achievement and Functional Performance) and goals; draft must be completed 3 days prior to the IEP meeting • Participate in Eligibility and IEP Meetings; Provide 1 week notice if not able to attend • As per request from the school principal, and with written parent permission, assist with Harm to Self and Harm to Others Assessments to make recommendations to administration • Serve on the school’s Crisis Response Team, including responding to Safe UT notifications • Conduct formal mental health assessments, as permitted under the evaluator’s license, to determine appropriate level of care, then develop and oversee the implementation of mental health plans for students • In collaboration with the principal, coordinate community resources to support the mental wellness of students including obtaining Release of Information (ROI) documents required by law • Participate in the Social Emotional Learning Support Team Professional Learning Community • Provide guidance on the appropriate delivery model of social emotional learning instruction for students who receive counseling services
