DEFINITION: Under general supervision deliver direct-care services to SUSD students’ and their families utilizing a community-based treatment modality. Provide intervention and support to families in need of services. Intervene with students and families in the schools and in the homes. Provides individual and group mental health support programs. Needs to be flexible in scheduling, as some evening or weekend work may be required in order to work with families.

TASKS: Completes AWARE professional training provided by ADE Department of Health and Human Services Substance Abuse and Mental Health Services Works with a district-level team of Mental Health professionals to provide services utilizing short-term, solution focused treatment strategies to students and families
Facilitates care coordination with other service providers to enlist existing supports in determining and meeting service objectives and to identify additional resources and supports
Receives referrals from schools requesting assistance for students and families; maintains records of cases and files; coordinates comprehensive services including referrals for medical, dental, vision, psychiatric, and mental health services; provides coordination between schools and outside placement agencies
Prepares written service plans and treatment update summaries, incorporating input from clients, other service providers, wraparound team members to identify problem areas and needs, service objectives, and intervention strategies
Provides direct clinical services to students, including individual and family interventions
Attends various meetings; develops on-going support groups; acts as liaison with outside agencies, schools, and programs; provides individual crisis counseling as needed; performs other duties as assigned. Completes progress reports in reference to service delivery within 48 hours
Participates in child specific Teacher Assistance Teams (TAT) and other interdisciplinary, diagnostic, or planning meetings
Links students and families with external programs or services, such as: health services, recreational activities, child care services, financial resources, housing placement, employment resources, child care, AA/NA groups, transportation resources, and others
KNOWLEDGE, SKILLS AND OTHER CHARACTERISTICS:
Knowledge of principles and techniques of youth and family interventions
Knowledge of services needed for culturally and economically diverse populations
Knowledge and skills in the development and implementation of service plans
Knowledge and skills in teaching mental health, anger management, and social skills
Knowledge in the area of child development, needs, behaviors; Crisis intervention and management.
Knowledge of community based service agencies.
Knowledge human services personnel roles and relevant community resources
Ability to maintain effective working relationships with students, families, and other service providers
Ability to communicate effectively and professionally, both orally and in writing
Skill in identifying the needs of individuals and families in the areas of educational, social, mental health, and medical services, then developing and implementing the appropriate services needed.
Candidate must meet eligibility for employment through successful criminal and central registry checks
PHYSICAL REQUIREMENTS: Work is performed in an indoor environment. Work requires frequent sitting, standing, bending, repetitive motion, vision to monitor and lifting up to 15 pounds.
Arizona Department of Education certification in either Counseling or Social Work.
Three (3) years of experience in the mental health or in a mental health or school setting, or a combination of experience, education, and training working with children and/or adolescents.
Experience in social services, mental health settings.
Experience working within schools.
Experience with bilingual (Spanish/English) populations
PREFERRED QUALIFICATIONS: Licensed Professional Counselor (LPC)
Depending on the needs of the District, some incumbents in this class must be able to demonstrate fluency in both Spanish and English as a condition of employment
