JOB DESCRIPTION
TITLE: Registered Nurse (Preferred); LPN; Health Aide
QUALIFICATIONS: 1) Ability to complete clerical forms and maintain files
2) Nursing/ Medical and/or First Aid training
3) Light typing/clerical work
CERTIFICATION: RN State License; LPN State License; High School Diploma or G.E.D. Certificate
REPORTS TO: Principal, Program Director, Social Worker and District Registered Nurse
JOB GOAL: To provide assistance to the school staff and social worker to maintain a safe environment and to facilitate referral and multi-disciplinary staff conference process.
PERFORMANCE
RESPONSIBILITIES: 1) Account for and maintain Pupil Personnel Services student records.
2) Assist social worker in preparation of all required forms and notices to staff in preparation for the district consultation system and annual review.
3) Assist social worker to maintain social work contact logs.
4) Assist in scoring and recording of student test and screening results.
5) Assist and conduct the reporting of data from district-wide screening results.
6) Maintain a working knowledge of clerical and operational procedures contained in the Pupil Personnel Services manual.
7) Maintain and update Pupil Personnel Services Manual.
8) Administer first aid to students and faculty.
9) Conduct tests and retests, and record results of vision and hearing screening for K, 1st, 2nd, 3rd, and 8th grade students and Special Education students, Early Childhood.
10) Supervise the administration of prescribed medication by students.
11) Participate in Early Childhood and pre-kindergarten screening.
12) Prepare and distribute a list of students with special health problems and/or needs.
13) Refer students who fail vision and hearing screening and provide necessary counsel with parents to assure necessary medical interventions.
14) Check medical, dental and immunization records of new students. As required for admission to school program.
15) Maintain up-to-date lists of students not adequately immunized and prepare yearly survey.
16) Maintain accurate health records on each student and record data on the computer.
17) Maintain student attendance and contact parents if the absence is not called in.
18) Assist secretary in general school office operation.
19) Other responsibilities as directed by the Building principal or program director.
SALARY: As determined by the Board of Education
EVALUATION: Performance will be evaluated annually in accordance with provisions of the Board’s policy on Evaluation of Classified Personnel
