Primary Location: Brewster Elementary School 

Qualifications:

  1. Completion of an accredited physical therapy program

  2. Current certification

  3. Licensed to practice in Connecticut

Reports to: Director of Student Services and Special Education

Job Goal: Provide physical therapy services to students in a school setting. Develop and execute service plans to improve, develop, restore or maintain students' active participation in self-maintenance, work, leisure, and play in educational environments.

Performance Responsibilities:

  1. Data collection and evaluation.

  2. Devise treatment/service plan and provide direct service according to the written plan.

  3. Recommend treatment modifications based on students' needs.

  4. Adapt environments, tools, materials and activities according to students' needs.

  5. Communicate and interact with other team members, school personnel, and families.

  6. Participate in meetings as a member of the multidisciplinary team.

  7. Travel to and among schools to provide services.

  8. Maintain treatment areas, equipment and supply inventory as the service plan requires.

  9. Maintain record of service provided including Medicaid reimbursement forms.

  10. Participate in policy and procedure development; participate in in-services and training.

  11. Other duties as may be assigned by Supervisor.

Terms of Employment:

Ten-month year (15 hours per week) and additional summer hours. 

Salary and work year to be established by the Board.
